Your Path to Recovery:

Assessment

A biopsychosocial, multidimensional Substance Use Assessment based on the ASAM Criteria conducted by a licensed professional to ascertain an individuals level of care needs for treatment of their substance use. The individual receives the assessment and the Substance Use Subtle Screening Inventory (SASSI A2, 4), an evidence-based screening instrument. Diagnosis and treatment recommendations are determined as a result

Telemedicine

During Covid-19 pandemic services are primarily delivered via telehealth (Zoom, Doxy Me Pro) with consent of the individual, using HIPPA compliant platforms to protect privacy.

Outpatient Services

ASAM Level 1.0

Individual/Group

Individual substance abuse counseling with a licensed professional as determined to meet individualized needs based on the ASAM Assessment. A licensed professional will develop an individualized treatment plan with the client. The number of sessions is based on the individuals needs and progress.

Family

The family meets with a licensed professional to explore addiction,  healthy relationships and recovery principals.

Prevention & Education

Group counseling for adolescents consisting of 10 one-hour sessions utilizing evidence based modalities and curriculums. Topics may include drug effects and education, issues encountered while learning about substance abuse and in early recovery, and ultimately how to maintain abstinence.

Intensive Outpatient

ASAM Level 2.1:

Substance Use Intensive Outpatient (SAIOP) services is a community-based program for adults (21+) and Adolescents (12-20 years old) with substance use disorders and/or co-occurring mental health disorders whose assessed substance use treatment needs fall within the ASAM level 2.1 continuum of care. This service is provided to individuals who do not require the intensive level of care of inpatient, residential, or partial hospitalization services, but require more intensive services than the weekly or bi-weekly sessions of Outpatient services.

IOP Adults

Substance Use Intensive Outpatient services provides an average of nine (9) hours per week.

IOP Adolescents

Substance Use Intensive Outpatient services provides an average of six (6) hours per week.

Mental Health Services:

MENTAL HEALTH EVALUATIONS

A Mental Health Evaluation (MHE) is used to ascertain whether or not an individual is functioning on a healthy social, emotional, and behavioral level, and to diagnose potential mental health disorders. The MHE includes a biopsychosocial assessment and the following assessment tools: Psychological Screening Inventory (PSI-2), Beck Depression Inventory, and Beck Anxiety Inventory. Diagnosis and treatment recommendations are provided. The MHE is conducted by a licensed therapist, or a licensed Resident or Supervisee under the direct supervision of a licensed therapist

MENTAL HEALTH INDIVIDUAL THERAPY

Individual mental health outpatient therapy with the individual by a licensed therapist or a licensed Resident or Supervisee under the direct supervision of a licensed therapist.  These services may be provided via Telehealth when clinically appropriate as determined by the treating therapist, and as covered by the individual’s insurance plan.

Family/Couples Therapy

Outpatient therapy with the family (or spouse), with or without the individual, by a licensed therapist or a licensed Resident or Supervisee under the direct supervision of a licensed therapist.

Group Therapy

Group therapy will be provided by a licensed therapist or a licensed Resident or Supervisee under the direct supervision of a licensed therapist. Goals established individually with the client for their ISP will be addressed in a group setting. Groups will have no more than 10 individuals at a time.

Opioid Treatment Services

Preferred Office-Based Opioid
Treatment (OBOT) services:

Provided by a buprenorphine-waivered practitioner
and a co-located licensed Credentialed Addiction Treatment Professional .

Assess the severity of the opioid use disorder
and prescribe medications such as Suboxone
when appropriate.

Preferred OBOT’s provide cognitive, behavioral, and other substance use disorder-focused therapies, by a Credentialed Addiction Treatment Professional, reflecting a variety of treatment approaches, provided to the member on an individual, group, or family basis.

Substance Use Care Coordination

Substance Use Care Coordination includes activities to ensure that necessary services are addressed through planning, monitoring progress, tracking outcomes and reporting back to the members of the interdisciplinary team. Care coordination includes connecting individuals with community resources, peer supports, medical, behavioral health, or social services needs outside
the practice.

Peer Supports

Peer Support Services for adults (21+) is a recovery oriented rehabilitative service for individuals provided by a certified Peer Recovery Specialist (PRS). A PRS is an individual with lived experience with substance use or co-occurring mental health and substance use disorders who has been successful in the recovery process. Specific Peer Support activities promote empowerment, self-determination, and understanding through mentoring, service coordination, and teaching positive coping skills to use when
